If your school or organization holds a valid state-issued sales tax exemption certificate, please complete the form below and send us a copy of the certificate. If you’re not sure if your organization is exempt from sales tax or where to find the certificate, please contact your school's administration or organization’s accounts payable office.

Please scan the valid certificate or take a photo and email a copy to orders@teachley.com.

Once you submit this form and we’ve received a valid, completed state-issued sales tax exemption certificate for your organization, please allow up to 10 business days for Teachley to update your organization’s account then issue a refund, if applicable.
